Who is a good candidate for rhinoplasty?

The procedure starts with the patient’s medical history taken. Following the physical examination, an appointment date is scheduled. This procedure, which is preferred by those who are unhappy with the shape of their nose, can also be applied to those with genetic nasal deformities. Finally, Rhinoplasty can be used for nasal deformities that may occur due to trauma or injuries.

Things you should know before a rhinoplasty

Prior to a rhinoplasty procedure, patients are briefed about some important details concerning the procedure. First of all, along with the tests to be performed, patients are examined for a possible common cold or flu. As with any other aesthetic procedure, the most important factor that patients should pay attention to during a rhinoplasty process is the pre-operative diet. Patients are asked to avoid any kinds of herbal supplements and blood thinners under any circumstances. Female patients usually inquire about the possibility whether a rhinoplasty procedure can be performed during menstrual period. Thanks to the recent developments in the medical field, it is possible for women to get a rhinoplasty during menstruation. It is also important that patients avoid smoking and drinking alcohol before the procedure.

How is the rhinoplasty procedure performed?

After the nasal structure of a patient who seeks to get a rhinoplasty, is examined and all the other necessary medical examinations are completed, the rhinoplasty procedure will start with an appointment date scheduled for the patient. Rhinoplasty surgery is a procedure that usually lasts up to 4 hours and is performed under general anesthesia. With an incision made in between the nostrils, nasal deformities are corrected. Nasal tip and bridge problems of patients, if any, are also addressed during the procedure.

Rhinoplasty procedures are performed without damaging the surgical area. And there is no risk of complications as the nasal bones or cartilage tissue are not damaged.

Post-operative instructions for rhinoplasty

No severe complications occur after aesthetic operations. Only a slight swelling that can last for a few days may be observed in the facial area. And since there will be muscle pains and headaches as well, they will be taken care of with the medicines prescribed by your doctor. Also in recent years, after a successfully completed rhinoplasty procedure, it is possible to get rid of bruises by applying the ointments as advised by your doctor.The only thing you need to do to address such problems, which can appear 24 hours after the procedure, is to use a high pillow before sleeping on the first night. Within a few days, changes in your nose will be noticeable and conditions such as bruising or swelling will disappear.

Recovery after rhinoplasty procedure

The recovery period after rhinoplasty procedures can completely be shortened with properly applied care and medical dressing. You can contribute to this healing process by following the instructions of your doctor at home. However, you should avoid touching the tampon placed in your nasal area in any way. At this stage, there will also be nasal splints and plasters around your nose. These procedures, which serve to retain the new shape of your nose, are performed only under the doctor’s control.

Avoiding any kinds of damage to your nasal area will also shorten the recovery period. Following the dressing applied on the 10th day of the procedure, patients are advised not to take aspirin and blood thinners. This instruction will be given to the patients soon after the procedure as well. As you can see, rhinoplasty procedures always help you achieve a more appealing facial appearance. And if you avoid smoking and drinking alcohol, you can ensure the recovery of nasal area in a short time and achieve a more aesthetically-appealing nose structure.

There is a difference between a male nose and a female nose. A male nose should be reshaped to achieve a natural look with a straight nasal bridge and a less elevated tip, while a female nose can have a little more curvy nasal bridge and a slightly more elevated tip. However, no rhinoplasty procedure should be planned in a way where nostrils will show in the end. The primary goal of the procedure is to achieve an aesthetically-pleasing and natural-looking result which is in harmony with the rest of patient’s facial features. Therefore, planning the procedure is just as important as the procedure itself and the final desired look of the patient and the surgeon’s capabilities should completely match.

Some patients may only have problems with the tip of their nose. In such cases, a procedure addressing only the tip of the nose can be applied. In the case of nasal tip ptosis, i.e. “a drooping nose”, the tip is elevated, while in the case of bulbous tip, i.e. “a round-shaped nose”, it is refined.

After the procedure is planned, the patient is briefed about other technical details related to the operation. First of all, the patient is advised to avoid using any medications such as blood thinners. At least 5 hours of fasting is absolutely necessary on the day of procedure. A pre-anesthetic checkup will be done and the patient will be examined by the anesthesiologist. If it is alright to proceed with the surgery, the procedure will start after a final consultation between the surgeon and the patient.

It is more preferable to perform a rhinoplasty procedure under general anesthesia. This way it can be more psychologically-reassuring for the patient and it can help with post-op pains. However, procedures involving only the tip of nose or other minor procedures can be performed under local anesthesia.

There may be a very slight swelling and bruising after the procedure. However, this situation varies between patients. Patient’s nasal structure, whether or not they have a surgical history, and their bone structure are the deciding factors here. There are three factors concerning post-op bruising and swelling. The first is a patient-related factor and can be referred to as the bleeding tendency of the patient. Despite all test results of the patient showing no signs of problem, the patient may have thin blood. In such cases, it is necessary to treat patient with utmost care. The second factor is the anesthesia. To avoid swelling and bruising, the procedure is performed with a slightly low blood pressure.  In this case, the duration of surgery is very critical. If the procedure takes longer than necessary, the patient may experience swelling and bruising. And finally, the last factor is for the treating surgeon to adopt a gentle and less traumatic technique. This way the patient will experience no post-op pain and less bruising.

As rhinoplasty procedures are performed under general anesthesia, there will be no pain during the operation. And because of a cast placed on the patient’s nose after the operation, the nose will be secured. For such reasons, patients will not experience any pain. Moreover any possible discomforts or patients with a low pain threshold in general  will be treated with oral painkillers. However, please note that a rhinoplasty surgery is considered as a pain-less procedure.

Ideally speaking, rhinoplasty procedures last 1 hour. This may vary if the patient has a surgical history. However, approximately 1 hour should suffice. The experience of the surgeon is also a decisive factor at this point.

The patient should be calm and comfortable on the day of procedure. Having a complete trust in your doctor will increase the success of the surgery significantly. If the patient is anxious, a sedative or tranquilizer drug can be administered to the patient before the surgery. This way, when the patient goes into the operating room, they will be anesthetized in a calm, comfortable and safe environment.

After being anesthetized, patient will be covered with a sterile medical drape. During the procedure, special medicines are administered to the patient to have a clearer access to the surgical area and to minimize swelling and bruising by limiting the bleeding. After 5-6 minutes following the administration of these medicines, surgeon proceeds with the operation.

Two different techniques can be applied in rhinoplasty procedures, i.e. closed and open techniques. Closed technique is more preferable for less problematic and straight noses with less nasal tip and breathing problems or for patients with less complaint. Open technique, on the other hand, is more preferable for patients with a surgical history, a relatively more crooked nose, severe breathing problems and more problematic nasal tip problems. The technique to be used can be determined before or during the procedure. Patient’s request will be taken into consideration as well, so long as the conditions are favorable and the final decision is made with the patient. The important thing here is the result and reshaping the nose to be in harmony with the rest of patient’s facial features without any problems.

After the operation, a cast is placed on the patient’s nose. Intranasal silicone splints can also be placed if necessary. However, nowadays, nasal tampons are no longer used for rhinoplasty operations. After the procedure, the patient awakens from anesthesia immediately. After waiting 5-10 minutes in the operating room and when the general condition of the patient is found to be good, they will be moved to their room.

The patient can be discharged after 5 hours in the hospital upon their request. However, they can also stay in the hospital overnight. In the latter case, a cold compress is applied to help with edema and swelling during the patient’s stay in the hospital. The patient can eat liquid foods 4-5 hours after the surgery and then return to their normal diet.

When the patient is discharged, an oral painkiller and antibiotics will be prescribed. Antibiotics are given as a preventive treatment. The patient should take the medicines twice a day, in the morning and in the evening. The patient is asked to schedule a follow-up appointment 6-7 days after the operation, where the cast on the nose and splint, if any, will be removed.

People who have had a rhinoplasty should avoid using glasses for at least six months after the surgery. Apart from this, the period until the nose realizes its full shape and definition, in other words, the period of full recovery will take one year, and therefore patients have to be patient. They will witness a steady progress in the shape of their nose during the recovery period. The slight swelling and edema improves in time as the nose keeps healing. As for breathing, there may be swelling inside the nasal passage, just like outside of the nose. Therefore, patients may need to use saline sprays containing sea or salt water. A thin skin-colored plaster is attached to the patient’s nose to ensure a faster recovery. It should be kept in place for about a week to help with the healing process and make it easier for the patient to return to their routines without any problems.

The only thing that patients should absolutely avoid after a rhinoplasty procedure is the use of glasses. They should not wear glasses for at least six months. After a month, the patient can go back to physical activities and go to fitness or gym. They can swim two weeks after the operation and can go out in the sun. There are two special nasal sprays the patient should start using 15 days after surgery. These sprays ensure a faster relief from nasal congestion and make it easier to breathe without causing any intranasal wounds.

Intranasal deviation problems of patients with nasal congestion problem can be corrected very quickly with the procedure. Since the nasal passage is opened, the smell receptors inside the nose will work better and the sense of smell will be improved. In this way, the patient will perceive an improved sense of smell and a better voice quality after the surgery, and hyponasal speech problem will be fixed.  Rhinoplasty procedures aim to improve your nose not only aesthetically but also functionally. Functional improvement refers to improvement in breathing, the sense of smell and voice quality. On the other hand, sinusitis is a common problem among people experiencing nasal congestion. Especially patients with sinusitis problems may have chronic headaches and a constant feeling of irritation or nasal flow at the back of the throat, i.e. nasopharynx. When the nasal passage is opened, all these problems will be solved and sinusitis gets better as well. If the patients has a chronic sinusitis problem, they must get a sinusitis surgery. However, first, it is necessary to open the nasal congestion that causes this problem in the first place.

Such concerns are a serious problem for many patients. The best thing to do at this point is to talk to your doctor about your concerns and have a better understanding of your own expectations and of the capabilities of the surgeon.
